Pathogenesis of Youth and Type 2 Diabetes and Prediabetes

The goal is to explore if genetic factors are associated with insulin resistance in children and and adolescents. Screening includes having an oral glucose tolerance test to measure blood glucose levels and insulin levels and to have a blood sample drawn for genetic testing. Individuals with the CC or TT genotype of the TCF7L2 gene and are have impaired glucose tolerance based on the oral glucose tolerance test are eligible to participate in the main study which includes an infusion study and other tests of glucose and insulin secretion.

Trial Purpose and Description

Type 2 Diabetes in obese youth is often preceded by a prediabetic state called: Impaired Glucose Tolerance (IGT), which is associated with a pre-existing defect in insulin secretion. We will determine here if genetic factors are associated with defects in insulin secretion and incretin system and hepatic insulin resistance in obese adolescents. Our long term goal is to generate information on both the genetics as well as the pathophysiology of Type 2 Diabetes in Youth, which ultimately might guide us towards better preventive and treatment avenues.

Eligibility Criteria

  • Inclusion
  • Both males and females are eligible between the ages of 12 and 18. They must be overweight or obese but have no other significant medical conditions (no kidney, liver or cardiac issues). Prediabetes is ok.
  • Exclusion:
  • We do not accept healthy volunteers. Participants must be overweight or obese with risk for prediabetes.
